Submit Pre-registration form below if you are:

1. A new family/person moving to or visiting Houston.
2. You are a new family member visiting or moving with an existing Houston family.
3. You are a visitor wanting to receive Jamaat communication and FMB Thali.

Valid ITS and HoF ITS numbers are required for all persons.

Please fill information for all family members as noted below.

• First Adult = Head of Family (HoF)
• Second Adult = Spouse of HoF
• Children = All additional family members (children/parents/grandparents etc.)
• If a joint family with two or more HoF then submit separate forms even if staying at the same address.
• A Houston area address is required for all visitors even if its a temporary one.
• If you are moving to Houston then you have to also Transfer your ITS to Houston Jamaat. Registering here does NOT automatically initiate an ITS transfer.
